## Privacy notice for health questionnaires
Laura Bonesi Nutrition uses online health questionnaires to gather additional information that may be relevant to your nutritional therapy consultations. This may include information about symptoms, health history, diet, lifestyle, environment, and other factors relevant to your case.
The information you provide in these questionnaires will be used only to inform your consultations, support clinical reasoning, and help tailor your nutrition and lifestyle recommendations. It will not be used for marketing purposes.
Your questionnaire responses will be stored securely and treated as confidential health information. They will form part of your client record and will be handled in accordance with the main Laura Bonesi Nutrition Privacy Policy, which you agreed to when starting work with me.
Your data may be stored using secure online systems used by Laura Bonesi Nutrition to collect, manage, and review client information. Access is limited to Laura Bonesi Nutrition and, where necessary, trusted service providers involved in delivering or maintaining these systems.
Questionnaire data will be retained in line with the main Privacy Policy, together with your client records, for up to 7 years from the date of your first appointment, unless a different retention period is required by law.
